About the Role:

Join GRACE AT WORK as a Case Manager RN in Bedford, IN, where you will play a vital role in providing compassionate care and support to our clients. Responsibilities:

Conduct comprehensive assessments of clients' health and social needs.

Develop and implement individualized care plans in collaboration with healthcare teams.

Monitor client progress and adjust care plans as necessary.

Provide education and resources to clients and their families.

Facilitate communication between clients, families, and healthcare providers.

Document all interactions and maintain accurate client records.

Advocate for clients to ensure they receive appropriate services and support.

Stay updated on best practices and regulations in case management and nursing.

Requirements:

Registered Nurse (RN) license in the state of Indiana.

Minimum of 2 years of nursing experience, preferably in case management.

Strong assessment, communication, and organizational skills.

Ability to work independently and as part of a team.

Experience with electronic health records (EHR) systems.

Compassionate demeanor with a client-centered approach.

Certification in case management (CCM) is a plus.

Willingness to engage in ongoing professional development.
