Position Summary
The In-Office RN Clinical Manager is responsible for the clinical oversight, supervision, and coordination of home health services to ensure safe, compliant, and high-quality patient care. This role provides day-to-day clinical leadership, ensures adherence to physician orders and plans of care, supports regulatory compliance with Medicare and Texas HHSC requirements, and serves as a key resource for clinical staff and agency leadership.
Key Responsibilities & Tasks
Clinical Oversight & Management
Provide clinical oversight of all home health services delivered by field clinicians
Ensure patient care is delivered in accordance with physician orders, plans of care, and professional standards
Review and approve clinical documentation, including assessments, visit notes, and care plans
Monitor patient outcomes and address clinical issues or variances in care
Serve as the clinical escalation point for patient care concerns
Staff Supervision & Clinical Support
Supervise, mentor, and support field clinicians (RNs, LVNs, therapists, aides, etc.)
Conduct clinical supervision, case reviews, and performance evaluations
Provide ongoing clinical education, coaching, and corrective action as needed
Ensure staff competencies, licensure, certifications, and required training remain current
Participate in hiring, onboarding, and orientation of clinical staff
Care Coordination & Interdisciplinary Leadership
Coordinate care across the interdisciplinary team, including nursing, therapy, social work, and physicians
Lead or participate in interdisciplinary team meetings and case conferences
Support transitions of care (hospital to home, recertifications, discharges)
Ensure timely communication of care plan updates and changes
Documentation & Regulatory Compliance
Ensure compliance with Medicare Guidelines of Participation, Texas HHSC regulations, and agency policies
Review OASIS assessments for accuracy, completeness, and timeliness
Ensure timely completion of physician orders, recertifications, and discharge documentation
Prepare for and participate in surveys, audits, and regulatory reviews
Address deficiencies, develop plans of correction, and monitor compliance improvements
Quality Improvement & Risk Management
Monitor quality indicators, outcomes, and adverse events
Identify trends, risks, or gaps in care and implement corrective actions
Oversee incident reporting, investigations, and follow-up actions
Participate in Quality Assurance and Performance Improvement (QAPI) activities
Support infection control and patient safety initiatives
Scheduling & Operational Support
Collaborate with administrative staff to ensure appropriate clinician coverage and visit frequencies
Assist with workload distribution based on patient acuity and staffing levels
Support timely start-of-care, recertifications, and discharges
Communicate staffing needs or operational challenges to leadership
Patient & Family Engagement
Address escalated patient or family concerns and complaints
Ensure patient and caregiver education is provided and documented
Promote patient satisfaction and continuity of care
Qualifications
Required
Active Registered Nurse (RN) license in the state of Texas
Minimum 2-4 years of nursing experience, with home health experience required
Prior experience in a supervisory or management role preferred
Strong knowledge of Medicare CoPs, Texas HHSC regulations, and home health operations
Current BLS/CPR certification
Preferred
OASIS and PDGM experience
EMR proficiency
Leadership or management training
Bilingual (English/Spanish preferred)
Physical & Work Requirements
Ability to work full-time in an office environment
Ability to sit, stand, and use a computer for extended periods
Occasional travel to patient homes or field visits as needed
Ability to lift up to 25 lbs
