About the Role:

ComForCare Home Health Care - Hilton Head is looking for a dedicated and compassionate Registered Nurse (RN) to deliver exceptional in-home care to patients in the Hilton Head, SC area. This is a rewarding opportunity to make a meaningful difference in the lives of patients and their families — right in the comfort of their own homes. Responsibilities:

Conduct comprehensive patient assessments and develop individualized care plans for home health clients

Administer medications, wound care, IV therapy, and other skilled nursing treatments in the home setting

Monitor and evaluate patient health status, documenting changes and communicating with physicians as needed

Educate patients and family members on disease management, medications, and home safety

Coordinate care with interdisciplinary team members including therapists, aides, and case managers

Ensure accurate and timely clinical documentation in compliance with home health regulations

Provide compassionate end-of-life and chronic disease management support as needed

Requirements:

Active Registered Nurse (RN) license in the state of South Carolina

Previous home health, hospice, or community nursing experience preferred

Strong clinical assessment and critical thinking skills

Excellent communication and patient education abilities

Reliable transportation and a valid driver's license for home visits in the Hilton Head area

Proficiency with electronic health records (EHR) and clinical documentation

Compassionate, self-motivated, and able to work independently in a home care environment
