Position Title: Nurse Consultant (Registered Nurse) PRN Reports To: Executive Director Location: Quest Inc. – 704 Metro Drive, Lebanon, PA 17042
Employment Type: Contract (Quarterly Requirement)
Position Summary
The Nurse Consultant provides nursing oversight and performs required quarterly assessments and service evaluations for clients of the Older Adult Daily Living Center (OADLC) as required under 6 Pa. Code §11.35 and § 11.123(2). The nurse supports staff education, promotes participant health and safety, and ensures compliance with applicable health standards and regulations.
This is a quarterly position involving on-site visits to complete core nursing functions for all participants.
Key Responsibilities (Per §11.123(2)(i)):
During each visit, the nurse shall:
Review the health status of each participant, including:
Vital signs (as appropriate)
Dietary needs and nutrition concerns
Chronic conditions or acute concerns requiring monitoring
Overall physical and functional well-being
Review medication procedures, if applicable:
Evaluate medication administration processes
Ensure medication storage and documentation procedures meet regulatory standards
Make recommendations to the administrator for improvements
Review personal care policies and procedures:
Assess hygiene, toileting, mobility, and transfer assistance protocols
Confirm that staff are following best practices and documentation is complete
Educate and support staff:
Provide in-service training or guidance as needed on:
Infection control
Chronic condition management
Emergency response procedures
Individualized care needs
Document training and staff participation
Document findings and recommendations:
Provide a written quarterly nursing report
Submit findings to the Program Director and retain for licensing compliance
Optional Duties (Per §11.123(2)(ii), if applicable):
(Only applicable if the center provides additional nursing services)
Observe participants for signs of medical instability
Support coordination of care with physicians or other providers
Qualifications
License: Current, unrestricted Pennsylvania license as either:
Registered Nurse (RN), or
Licensed Practical Nurse (LPN) under the supervision of an RN or physician
Experience: Minimum 1 year working with older adults or adults with functional impairments (required)
Clearances:
Ability to provide all background checks, RN license upon hire.
Knowledge:
Familiarity with aging-related health conditions, medication management, personal care practices, and adult day program environments
Understanding of Pennsylvania Chapter 11 OADLC regulations is preferred
Working Conditions & Expectations
Must be able to complete a full on-site review of all enrolled participants once per quarter (every 3 months)
Must prepare and submit a written documentation for each visit
Work is typically scheduled during normal business hours, 8:30am-2:30pm with flexibility for center needs
May be required to assist in staff education or policy updates
Annual trainings will also be required for the requirements of this role.
