We are a growing hospice and palliative care program and have an exciting opportunity for a Full Time RN Case Manager. This position will cover patients in NW San Antonio out to Boerne and parts of the Hill Country.We serve patients in their Home, Personal Care Homes, Assisted Living and Long Term Care facilities. One year hospice experience strongly preferred.

Salary plus additional pay available for On Call and weekends.
