Responsibilities: • Develop and implement nursing policies and procedures for home health services to ensure compliance with healthcare regulations and standards. • Oversee and manage the clinical team, including nurses and support staff, to provide high-quality patient care and support. • Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care plans and ensure effective communication within the healthcare team. • Monitor and evaluate patient outcomes and satisfaction levels to identify areas for improvement and implement strategies to enhance the quality of care provided. • Provide leadership and mentorship to nursing staff, fostering a positive work environment and promoting professional development and growth among team members. Additionally: • Consult, plan, and work with the Administrator in policy formulation and programmatic development and implementation including assessing adequate and appropriate staff. • Evaluates the program, services, and field personnel. • Directs the implementation of service goals and objectives. • Develops standards that ensure safe and effective services to clients and families. • Establishes job functions, qualifications, and job descriptions for each service staff (direct and supervisory). • Keeps program personnel up to date with state-of-the-art clinical information and practices staff development including orientation, in-service education, and continuing education. • Maintains policy and procedure manuals and updates procedures and job descriptions as policies and procedures change. • Directs staff in the performance of their duties. • Oversees the implementation and evaluation of client care services inclusive of regulatory licensure and certification criteria and accreditation standards. • Assesses appropriate staff supervision during all service hours. • Assists in the evaluation of community needs and plans programs and services to meet identified needs. • Assists in agency evaluation activities. • Assists in marketing the agency and fostering community relations. • Makes patient and personnel assignments. • Coordinates patient care. • Coordinates referrals. • Ensures the development, implementation, and updates of the individualized plan of care. Qualifications: • Active RN license in the state of Virginia. • MUST HAVE: 2+ years of experience in home health. • MUST HAVE: 1+ years of supervisory experience in home health. • Must be able to pass a criminal history background check. • A Bachelor’s degree is preferred. Compensation: $90,000 yearly
