Essential HealthCare Solutions is seeking a skilled Registered Nurse to join our team, for (Referral Coordination Team).  Candidate must be committed to providing high-quality patient care. The ideal candidate will have experience in various nursing practices and will be responsible for performing both direct and indirect nursing care, contributing to patient assessments, determining care priorities, and evaluating outcomes. The RN will demonstrate strong independent judgment, organizational skills, and flexibility, while utilizing an interdisciplinary approach to deliver comprehensive care. This position requires a proactive, compassionate individual who can collaborate effectively with patients, families, and healthcare teams to ensure optimal patient outcomes.

Responsibilities

• Triage incoming referrals and discuss available care options.
• Provide scheduling guidance and be a resource within the service.
• Guide Veterans through their full range of care options including internal/direct care in VA and care in the community.
• Utilize a national standardized triage tool.
• Serve as the liaison between ordering or referring providers and specialty care services.
• Confirm the appropriate care is requested and all necessary pre-work and tests are completed.
• Comply with hospital mandated use of health information solutions.
• Work collaboratively with the Office of Integrated Veteran Care to triage Request for Services (RFS).
• Utilize delegation of authority to function at the highest level of role performance.
• Align high-quality care with key performance indicators.
• Support colleagues and other nurses through knowledge sharing.
• Evaluate outcomes of evidence-based decisions and practice changes.

Requirements

• Valid Registered Nurse (RN) license in the state of practice.
• Minimum of two (2) years direct patient care experience within the last three (3) years.
• Proof candidate a graduate of an ACEN or CCNE accredited program iv. Copies of All Licensure and Board Certification and All Primary Source
• Can hold an Active/unrestricted license in a State, Territory, or Commonwealth of the U.S. or in the District of Columbia
• Copy of BLS (American Heart Association preferred)Excellent communication skills and the ability to work collaboratively within a healthcare team.
• Commitment to maintaining patient confidentiality and adhering to HIPAA standards

Pay Rate: $45-$55 per hour
