Description
Experienced Registered Nurse (RN) with a demonstrated background in both direct patient care and leadership to supervise and direct nursing and other therapeutic services. The ideal candidate will have at least two (2) years of nursing experience, with a minimum of one (1) year in a supervisory or administrative role. This position ensures the highest standards of care are met and provides critical support to staff and patients alike.

Responsibilities/Activities:
· Supervise and direct the activities of nursing and other therapeutic staff to ensure quality patient care and compliance with clinical protocols.
· Provide guidance, mentorship, and support to nursing and therapeutic staff, fostering a collaborative and professional environment.
· Serve as a resource for staff by answering clinical and administrative questions, and offering clear, practical solutions.
· Resolve issues and concerns raised by staff, patients, or families within a reasonable timeframe, taking into account the urgency and nature of each situation.
· Evaluates patients referred to for home care to determine appropriateness for admission.
· Consults with family, institutions, physicians, patients, nurses, and others, while establishing a coordinated home health care plan.
· Completes OASIS nursing assessment, written plan of care (485), and completed assessment.
· Reviews nursing care plans/complete OASIS requirements and update as required.
· Reviews physician's plan of treatment, orders, and ensures execution.
· Completes a written report to physician as needed, with Plan of Treatment summary.
· Provides supervision for CHHA monthly as ordered

¨ This role requires a balance of hands-on patient care, staff supervision, and administrative oversight.
¨ The RN Supervisor must be adaptable, approachable, and responsive to both routine and unexpected clinical challenges.
Qualifications:
· Current and valid Registered Nurse (RN) license.
· Minimum of two (2) years of nursing experience.
· At least one (1) year of supervisory or administrative nursing experience.
· Proven ability to remain calm and decisive in emergency situations.
· Strong communication, leadership, and organizational skills.
· Ability to be on premises or reachable at all times while on duty.
· Traditional Medicare and Medicaid experience a plus
· Proficiency in performing procedures such as phlebotomy, venipuncture, tube feeding, and airway management.
· Understanding of medical terminology, ICD coding (ICD-9 & ICD-10), and NCQA standards.
