Unit: Adult Home Health

Job Duration: 13 weeks
Schedule: Day shift, 5x8 hours
Job Requirements & Qualifications:
- Minimum 2 years of RN experience, including recent adult home health or hospice experience
- Must have recent charting experience with HomeCare HomeBase; proficiency in OASIS required
- AHA BLS certification required
- Strong skills in clinical assessment, care coordination, wound care, medication management, and patient education
- Must have reliable transportation and current driver’s license
- Willing to travel up to 75 miles from home clinic
- On-call participation required
Patient Care / Documentation Responsibilities:
- Start of Care, Routine Visits, Significant Change, Resumption of Care, Discharge, Recertification
- Wound care, including drain management, pressure ulcer prevention and staging, V.A.C. management
- Medication administration and management (IV, IM, Subcutaneous, PCA pump, etc.)
- Gastrointestinal care (G-tube, NG tube, feeding tube management, colostomy/ileostomy)
- Genitourinary care (catheter insertion/management, post AV fistula/shunt)
- Respiratory care (nebulizer, oxygen, tracheostomy, ventilator management)
- Orthopedic and oncology patient care as applicable
Work Details:
- Visits both in patient homes and care facilities
- First-timers in home health not accepted; recent specialty experience required
Job Overview:
Provides, coordinates, and directs home nursing care according to physician orders and agency policies. Responsible for supervising and evaluating Home Health Aides and LPNs/LVNs while delivering high-quality patient-centered care.
