Responsibilities: • Coordinate and manage patient care plans, ensuring each individual's needs are met with compassion and precision. • Conduct regular home visits to assess patient conditions, providing hands-on care and symptom management. • Collaborate with an interdisciplinary team to develop and implement personalized care strategies. • Educate patients and families on care plans, fostering understanding and involvement in the care process. • Maintain accurate and timely documentation of patient interactions and care plans. • Advocate for patient needs, ensuring they receive the highest quality of care and support. • Participate in team meetings, contributing insights and updates to enhance patient care outcomes. Qualifications: • Experience in hospice or palliative care, demonstrating a deep understanding of patient-centered care. • Ability to coordinate and manage comprehensive care plans, ensuring each patient's needs are met with precision and empathy. • Proven track record of conducting thorough home visits, assessing patient conditions, and providing timely interventions. • Strong collaboration skills to work effectively with physicians, social workers, and other healthcare professionals. • Excellent communication skills to educate patients and families on disease processes and self-care techniques. • Proficiency in maintaining accurate and detailed patient records, ensuring compliance with healthcare regulations. • Advocacy skills to champion patients' rights and needs, fostering a supportive and respectful environment for all involved. Compensation: $53 - $56 hourly
