Responsibilities: • Coordinate and manage patient care plans, ensuring they align with each individual's unique needs and preferences. • Provide compassionate, hands-on nursing care to patients in their homes, focusing on comfort and quality of life. • Collaborate with an interdisciplinary team to develop and implement comprehensive care strategies. • Educate patients and their families on symptom management, treatment options, and end-of-life care. • Conduct regular assessments to monitor patient progress and adjust care plans as necessary. • Serve as a primary point of contact for patients and families, offering emotional support and guidance. • Ensure timely and accurate documentation of patient care activities and outcomes in compliance with regulatory standards. Qualifications: • Experience in hospice or palliative care, with a deep understanding of end-of-life care principles. • Ability to conduct thorough patient assessments and develop individualized care plans. • Proven track record of effective collaboration with interdisciplinary teams to enhance patient outcomes. • Strong communication skills to educate and support patients and families, fostering a compassionate care environment. • Proficiency in maintaining accurate and timely documentation, ensuring seamless care transitions. • Ability to advocate for patient needs, ensuring they receive comprehensive and empathetic care. • Flexibility to manage a dynamic caseload, adapting to the unique needs of each patient and family. Compensation: $53 - $56 hourly
