    Job Title: Registered Nurse – Home Health/Hospice

    Location: Leominster, MA

    Shift: Day 5x8-Hour (8:00 AM – 5:00 PM)

    Start Date: 04/06/2026

    Job Duration: 13 weeks

    Department: Hospice

    Job Overview: Provides and coordinates home nursing care according to physician orders and agency policies, applying the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and LPNs/LVNs. Home visits required; must have reliable transportation, current driver’s license, and agency-required liability insurance. Average 5–8 patients per day.

    Responsibilities:

    • Perform start-of-care, follow-up, and wound care visits for hospice patients.
    • Administer medications, manage pain using WHO analgesic ladder, and provide palliative care interventions.
    • Assess symptoms including altered mental status, anorexia/cachexia, nausea/vomiting, pressure ulcers, and more.
    • Start and maintain IVs; care for peripheral IVs, central lines, and ports.
    • Provide patient/family education, bereavement support, and cultural/spiritual care.
    • Ensure compliance with ethical and legal guidelines, advance directives, and Medicare regulations.
    • Document accurately in HomeCare HomeBase (HCHB) and OASIS EMR systems.

    Requirements:

    • RN license required in MA.
    • Minimum 2 years of experience, preferably in Hospice or Home Health.
    • BLS/CPR certification required.
    • Proficiency in HCHB and OASIS charting systems.
    • Must be able to travel up to 30 miles from home clinic.

    Work Settings: Home Health/Hospice, Inpatient Hospice, Pediatric Hospice, Skilled Nursing

    Certifications/Skills: Pain management, symptom assessment, medication administration, IV/Phlebotomy skills, patient/family education, cultural awareness, end-of-life care
