    What makes this role different

    No hands-on care

    No clinical procedures

No lifting, transfers, bathing, or personal care

Conduct verbal and observational assessments with clients and family members

Review medical history, diagnoses, medications, functional abilities, and daily routines

Identify risks, support needs, and appropriate non-medical services

Develop and update individualized care plans for in-home caregivers to follow

Serve as a clinical resource to the care team regarding the client’s plan of care

Maintain accurate and timely documentation of assessments and care plans

The focus of each visit is understanding the client’s health background, functional needs, and personal goals in order to build a realistic and supportive care plan.

This position works well for nurses who:

Want to step away from physically demanding clinical environments

Prefer predictable, scheduled visits rather than shift-based floor work

Value meaningful, one-on-one conversations with clients and families

Enjoy care coordination, planning, and education

Want to continue using nursing judgment without bedside responsibilities

    Qualifications

    Active Registered Nurse (RN) license

    Strong documentation and care-planning skills

Comfortable working independently in the community

Professional, compassionate communication style with clients, families, and caregivers

    Work environment
