We Are Hiring

Registered Nurse – Home Health

Our RNs provide skilled nursing visits, patient assessments, care coordination, medication education, wound care, disease management, post-hospital follow-up, and support for patients recovering from illness, surgery, injury, or a change in condition.

RN responsibilities may include:

Skilled nursing visits in the patient’s home

Start of care, recertification, resumption of care, and discharge assessments

OASIS documentation and care planning

Medication reconciliation and patient education

Wound care and chronic disease management

Communication with physicians, families, therapists, and the care team

Helping patients remain safe, independent, and confident at home

Licensed Practical Nurse – Home Health

Our LPNs provide hands-on skilled nursing care under the direction of the RN and physician plan of care. This role is ideal for a nurse who enjoys patient education, strong clinical follow-up, and building relationships with patients and families in the home setting.

LPN responsibilities may include:

Skilled nursing visits in the patient’s home

Medication education and treatment follow-up

Wound care and vital sign monitoring

Support for chronic disease management

Documentation of patient progress and changes in condition

Communication with the RN case manager and clinical leadership team

Helping patients stay safe and supported at home

Requirements

Active Virginia RN or LPN license

Current CPR certification

Reliable transportation

Valid driver’s license and auto insurance

Home health experience preferred, but we are willing to train the right candidate

OASIS experience preferred for RN applicants
