    ComForCare Home Care is excited to expand our mission by hiring a PRN RN to join our Team.     Not every RN role has to mean long shifts and burnout! The nature of home care allows for one-to-one attentiveness; rather than rushing from one case to the next, you will be able to take your time with a set number of patients and provide exceptional care you can be proud of. This role is for someone who is self-directed, comfortable taking initiative, and open to learning as we build something different.     Requirements:

    Unencumbered New York State RN License

    CPR, BLS Certification

    2 or more years of hospital experience (Acute Care) and/or Home Care

    Ability to commute to Brooklyn and Manhattan for occasional assessments; infrequently, Queens and the Bronx

    Willingness to be part of a growing Team

    Duties:

    Perform comprehensive in-home assessments. This will include a detailed physical and psychosocial assessment, medication profile, and evaluation of the living environment. You will assist in implementing fall precautions, instructing on proper use of necessary supplies/equipment, and any other relevant safety measures;

    Develop or update a fully individualized plan of care based on your assessment;

    Perform Supervisory visits with established HHA/PCA and Client matches;

    Communicate with the office, caregivers, clients/families, to ensure everyone is updated on any developments or needs;

    Provide ongoing education and in-field support to HHAs and PCAs.
