Registered Nurse - Pediatric In Home Care - GAPP Program
The Southern Touch Home Care is seeking compassionate, experienced, and dependable Registered Nurses (RNs) to join our growing team serving medically fragile children through Georgia's GAPP (Georgia Pediatric Program).
As a GAPP RN, you will provide one-on-one skilled nursing care in the comfort of the client's home while helping families maintain a safe and supportive environment for their child.

About the GAPP Program
The Georgia Pediatric Program (GAPP) provides medically necessary skilled nursing and personal care services to medically fragile children under the age of 21 who require ongoing healthcare support in their homes.
Our clients may have diagnoses including:
Cerebral Palsy
Global Developmental Delay
Down Syndrome
Seizure Disorders
Respiratory Disorders
Tracheostomy Dependence
Feeding Tube Dependence (G-Tube/PEG)
Neuromuscular Disorders
Congenital Conditions
Developmental Disabilities
RN Responsibilities
Perform comprehensive nursing assessments
Develop and implement individualized nursing care plans
Administer medications and treatments as ordered
Monitor and document client status and progress
Provide tracheostomy care and suctioning when applicable
Manage G-tube/PEG tube feedings
Monitor and respond to seizure activity
Perform respiratory assessments and oxygen monitoring
Educate and support family caregivers
Complete required nursing documentation accurately and timely
Collaborate with physicians, therapists, case managers, and the The Southern Touch Home Care clinical team
Participate in supervisory visits and care plan reviews
Qualifications
Required
Active Georgia Registered Nurse (RN) License
Current BLS/CPR Certification, PALS preferred
Valid Driver's License and Reliable Transportation
Ability to pass GCHEXS Background Screening
Ability to work independently in a home setting
Strong communication and documentation skills
