We are currently seeking a Private Duty Registered Nurse to join our team. The Private Duty RN's overall responsibility is to provide direct patient care and case management to assigned clients in the home and facility setting. This is a full time position with fluctuating and flexible hours depending on client needs. The RN will provide care in patient homes and facilities throughout the towns we serve (Fairfield County).
Duties of Private Duty RN: • Provides direct patient care to patients of all ages. • Identifies and documents patient and family needs through comprehensive assessment. • Performs evaluation of patients’ progress, promptly acts on any changes made to patient’s plan of treatment or condition. • Updates and maintains the nursing care plan. • Appropriately utilizes and maintains all nursing equipment and supplies needed for patient care. Interprets findings of tests to physician, patient, or family as appropriate. • Counsels patients and families on the basic principles of wellness including information on related fields such as nutrition, safety, education, mental health, dental care, and overall health and wellness aspects. • Maintains case load and productivity. • Attends annual mandatory In-services including Blood-Borne Pathogens and TB in-services. Obtains minimum of 12 hours in-service annually, maintains CPR certification and seeks professional self-development. • As needed, covers evening, weekend and holiday time for the agency. • Assesses patients at risk for existing or potential situations of neglect and initiates referral to appropriate community agencies. • Ability to maintain Universal Precautions, infection control and proper bag technique. • Knowledgeable and compliant with all agency and regulatory procedures. • Completes quality documentation within required agency and/or regulatory timeframes. • Ability to establish respectful, positive working relationships with other employees and volunteers in the delivery of patient services. • Participates in total quality improvement process for both Agency and self including mandatory meetings. Accepts and utilizes supervision constructively in order to enhance the quality of agency documentation and professional self-development.
Qualifications for Private Duty RN: • Graduation from an accredited school of nursing. • Currently licensed to practice as a Registered Nurse in the State of Connecticut. • A minimum of one year of nursing experience, preferably in hospital setting. Home Health Care experience preferred. • Ability to drive throughout geographic territory (Fairfield County).
