Position Overview

We are seeking a dedicated Registered Nurse (RN) to perform initial home care assessments, wound care, develop personalized plans of care, and conduct evaluations prior to aide placement. This role is critical in ensuring our clients receive safe, effective, and tailored care from the start.

Responsibilities

Conduct in-home assessments of new clients.

Develop individualized plans of care in compliance with agency and state guidelines.

Evaluate client needs and environment prior to assigning home health aides.

Collaborate with care coordinators and families to ensure continuity of care.

Maintain accurate documentation and follow-up evaluations as needed.

Qualifications

Active RN license in Pennsylvania.

Minimum 1 year of experience in home health, geriatric care, or community nursing.

Strong assessment and documentation skills.

Reliable transportation for home visits.

Compassionate, professional, and detail-oriented.
