Job Summary

We are seeking a Registered Nurse to join our team for a Supervisory Visit and out of town Nursing assessments for Structured Family Care Clients and caregivers. We are needing a nurse in North West Georgia, North East Georgia, South West Georgia, South East Georgia. We cover over 51 counties for Structured Family Caregiving services.

Responsibilities

Completing 3 and 6 Month in person Visits while on the phone with members case manager also form given for completion

Working with our Medicaid Nurse who oversees all 3 month and 6 month supervisory visit

Health Coach Team Meetings

Qualifications

High school diploma/GED

Valid Nurses Licenses

First aid and CPR-certified

Basic computer skills

Ability to adhere to all health and safety guidelines

Excellent communication and interpersonal spills

Ability to lift heaving objects

Valid driver’s license and reliable transportation

Ability to work with limited supervision

Compassionate, respectful, ethical
