Job Summary
We are seeking a Registered Nurse to join our Hospice team. In this role, you will be managing a plan of care for patients receiving end of life care. You are a Registered Nurse (RN) who operates with great attention to detail, completes accurate and timely documentation, ensures effective communication and teaching skills for our patients, families and facility partners. To succeed, you should be able to demonstrated knowledge and skill in current nursing practices, end of life symptom and pain management, hospice guidelines and conditions of particpation, effective problem solving and have a big heart.
Responsibilities
Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
Ability to perform as a team player
Local travel to visit patients in the natural home or care facility ( primary service area is northern sector of the Dallas metroplex)
Assess patient status at each visit and monitor patients for change in symptoms or Change in condition
Document each patient assessment visit, consult team physician as needed and report to interdisciplinary team on changes as needed
Provide effective communication with families and facility caregivers regarding patient status at each visit.
Qualifications
Currently Texas Registered Nurse (RN) license
Previous experience as a Registered Nurse is preferred in the area of hospice
Excellent interpersonal and client care skills
Valid driver’s license
CPR certification required
