We are seeking a compassionate and experienced Registered Nurse (RN) Case Manager to provide skilled nursing care and case management services to patients in their homes and community settings. This role is ideal for a dedicated nurse who enjoys working independently while collaborating with physicians, caregivers, and interdisciplinary healthcare professionals to improve patient outcomes and quality of life.
As an RN Case Manager, you will assess patient needs, develop individualized care plans, coordinate services, and provide skilled nursing interventions that promote health, safety, and independence.
Key Responsibilities
Patient Care & Clinical Services
Conduct comprehensive nursing assessments, including initial, annual, follow-up, and reassessment visits.
Perform skilled nursing visits and complete OASIS assessments.
Develop, implement, and revise individualized Plans of Care (POC).
Monitor patient conditions and respond appropriately to changes in health status.
Provide medication management and patient/caregiver education.
Conduct home safety assessments and identify risks affecting patient care.
Evaluate ADLs, IADLs, and need for Home Health Aide services.
Care Coordination
Collaborate with physicians, therapists, caregivers, and community resources.
Coordinate interdisciplinary services to ensure continuity of care.
Participate in case conferences and care planning meetings.
Educate patients and families regarding treatment plans, services, and available resources.
Leadership & Supervision
Supervise Home Health Aides (HHAs), Licensed Practical Nurses (LPNs), and other assigned personnel.
Provide guidance, mentoring, and support to clinical staff.
Assist with orientation and training of newly hired nursing personnel.
Documentation & Compliance
Complete timely and accurate clinical documentation, including visit notes, OASIS assessments, care plans, and progress reports.
Ensure compliance with agency policies, HIPAA requirements, and state/federal regulations.
Participate in quality assurance and performance improvement activities.
Qualifications
Required
Graduate of an accredited nursing program.
Active Registered Nurse (RN) license in the State of Indiana.
Current CPR certification.
Minimum of one (2) year of nursing experience.
Valid driver's license, reliable transportation, and proof of insurance.
Ability to pass a criminal background check.
Preferred
Home Health experience.
Case Management experience.
Experience with Medicaid Waiver programs and Home & Community-Based Services (HCBS).
OASIS documentation experience.
Skills & Competencies
Strong clinical assessment and critical thinking skills.
Excellent communication and patient education abilities.
Knowledge of home health regulations and care planning.
Ability to work independently and manage a caseload effectively.
Proficiency with EMR and electronic documentation systems.
Work Environment
Primarily conducted in patient homes and community settings.
Frequent local travel required.
