    Key Responsibilities

    Develop and implement individualized patient care plans

    Perform in-home assessments and ensure quality care delivery

    Administer medications, IV infusions, and treatments as prescribed

    Provide advanced wound care, including Wound VAC (NPWT) management

    Maintain accurate documentation and ensure regulatory compliance

    Supervise and support nurses, aides, and caregivers

    Coordinate staffing and ensure proper coverage for patient cases

    Collaborate with physicians and healthcare professionals

    Qualifications & Requirements

    Must Have:

    Active and unrestricted Registered Nurse (RN) license

    Current CPR Certification

    2–3 years of clinical experience (home health, hospice, or skilled nursing preferred)

    Strong clinical, communication, and organizational skills

    Preferred:

    Reliable transportation, valid driver’s license & auto insurance

    Experience with IV infusion therapy and complex care cases

    Wound care expertise (including NPWT/Wound VAC)

    Leadership or supervisory experience

    Ability to handle on-call responsibilities and urgent situations
