Position Summary We are seeking an experienced Registered Nurse (RN) to conduct client assessments, develop individualized care plans and ADL support plans, monitor client progress, and evaluate caregiver performance to ensure quality care.

Responsibilities

Conduct in-home client assessments and evaluate living environments

Develop and update individualized care plans and ADL plans

Monitor client progress and adjust care plans as needed

Educate and support clients and families

Evaluate caregiver performance and provide feedback

Collaborate with caregivers and healthcare professionals

Maintain accurate documentation and records

Ensure compliance with healthcare regulations and standards

Participate in ongoing training and professional development

Qualifications

Active South Carolina RN license

1–2 years of nursing experience (home care preferred)

Strong assessment and clinical skills

Excellent communication and interpersonal skills

Experience developing individualized care plans

Ability to work independently and manage time effectively

Compassionate, patient-centered approach

Valid driver's license and reliable transportation
