Job Summary
Just Like Home is seeking a compassionate and experienced Registered Nurse (RN) to support individuals with intellectual and developmental disabilities enrolled in the Home and Community-based Services (HCS) Program. The RN is responsible for overseeing medical services, ensuring compliance with Texas Health and Human Services Commission (HHSC) regulations, coordinating healthcare needs, and supporting direct care staff in providing safe and effective services.
The RN will work closely with the Program Director, Case Managers, Direct Support Staff, physicians, pharmacies, and other healthcare providers to ensure individuals receive high-quality, person-centered care.
Essential Job Duties
Nursing Oversight
Conduct comprehensive nursing assessments for individuals receiving HCS services.
Develop, review, and update Nursing Care Plans as required by HCS program standards.
Monitor and evaluate individuals’ health status and respond to changes in condition.
Provide clinical oversight for medication administration and health-related supports.
Medical Coordination
Coordinate medical appointments, treatments, and follow-up care with physicians and specialists.
Communicate with pharmacies regarding medications, refills, and medication changes.
Ensure physician orders are properly documented and implemented.
Medication Management
Review Medication Administration Records (MARs) for accuracy and compliance.
Train and monitor Direct Support Staff on medication administration and health-related tasks.
Investigate medication errors and implement corrective actions.
Staff Training & Support
Provide nursing training and delegation to direct care staff as allowed under Texas Nurse Practice Act.
Conduct competency evaluations for medication administration and health procedures.
Provide guidance to staff regarding infection control, health monitoring, and emergency procedures.
Documentation & Compliance
Maintain accurate nursing documentation in compliance with Texas HHSC HCS regulations.
Ensure all medical records, physician orders, and nursing notes are properly maintained.
Assist with HHSC surveys, audits, and compliance reviews.
Health Monitoring
Monitor chronic medical conditions such as diabetes, seizures, hypertension, and behavioral health concerns.
Ensure proper implementation of specialized medical protocols and physician orders.
Review incident reports related to health and safety.
Qualifications
Required
Current Registered Nurse (RN) license in the State of Texas
Minimum 1 year nursing experience
Valid Texas Driver’s License
Ability to pass background checks required by HHSC
Strong documentation and communication skills
Preferred
Experience working with IDD / intellectual and developmental disabilities
Experience with Texas HCS or Medicaid waiver programs
Knowledge of HHSC regulations and standards
Skills & Competencies

Work Environment
Travel between group homes, day habilitation programs, and medical offices
