Primary Duties:

• Provide in-home skilled nursing services
• Evaluate patient health status and report concerns appropriately
• Coordinate care with physicians and interdisciplinary teams
• Educate patients and families on treatment plans and wellness goals
• Ensure all documentation meets regulatory standards

Qualifications:

• Current RN license in Arizona
• Reliable transportation for travel throughout the service area
• Excellent organizational and interpersonal skills
• Home health experience preferred but not required
