We are looking for dedicated and experienced Registered Nurses (RNs) to conduct client assessments on a per diem basis. If you have a passion for senior care, a strong clinical background, and the ability to provide personalized care, we’d love to hear from you!

Shifts: Assessments range from 1–3 hours.

Key Responsibilities

As a Registered Nurse with LOES, you will play a vital role in ensuring exceptional care for our clients. Your responsibilities will include:

· Initial Assessments: Completing comprehensive evaluations for new clients to determine care needs.

· Routine Home Visits: Monitoring and updating client care plans during monthly visits.

· PRN (As-Needed) Visits: Responding to client needs with timely assessments.

· Post-Hospital Assessments: Ensuring smooth transitions from hospital to home care.

· Recertification Assessments: Reassessing care needs every six months to ensure optimal support.

· Supervision of Home Health Aides: Overseeing aides during visits to ensure compliance with care plans and quality standards

Skills

· Strong clinical experience in various settings such as hospitals, nursing homes, and primary care.

· Knowledge of geriatric and long-term care best practices.

· Excellent communication and interpersonal skills to collaborate with clients, families, and healthcare teams.

· The ability to work independently while managing priorities effectively.

· A compassionate approach to care, prioritizing the well-being and dignity of clients.

Qualifications:

A valid NJ State RN License.

At least 2 years of experience as a Registered Nurse.

Current BLS and CPR certifications.

Fully vaccinated against COVID-19.

Previous home care experience is a plus but not required
