Job Summary: As a Chronic Care RN – Endocrinology & Diabetes Educator, you will play a key role in supporting patients through education, counseling, and ongoing care management, with a special focus on diabetes and metabolic health. You will work alongside providers in our Temecula Clinic to empower patients through individualized care plans and coordinated chronic disease management.
Key Responsibilities:
Diabetes & Endocrinology Management: Provide one-on-one and group education to patients with diabetes and endocrine disorders, covering topics such as blood glucose monitoring, insulin use, lifestyle changes, and complication prevention.
Nutrition Counseling: Develop and implement customized nutrition plans for patients with diabetes, obesity, hypertension, and other chronic conditions.
Patient Education & Empowerment: Educate patients and caregivers on self-management techniques, including medication adherence, dietary strategies, and exercise.
Care Coordination: Collaborate with physicians, nurse practitioners, and other care team members to ensure seamless management of chronic conditions.
Screening & Monitoring: Conduct clinical assessments including A1c monitoring, foot exams, and lifestyle risk evaluations.
Documentation: Maintain detailed, timely records of patient interactions, care plans, education sessions, and clinical outcomes in accordance with compliance standards.
Community Outreach: Participate in health education programs, support groups, and wellness workshops within the community or assisted-living settings.
Patient Advocacy: Serve as a supportive resource for patients, helping them navigate challenges and achieve their health goals.
Qualifications:
Education: Bachelor's degree in Nursing (BSN) required. Master’s in Nutrition or related field preferred.
Licensure: Current RN license in the state of California. Certified Diabetes Educator (CDE) and Licensed Nutritionist or Registered Dietitian credentials required.
Experience: Minimum of 1 years of RN experience in an endocrinology, diabetes management, or internal medicine setting.
Clinical Expertise: Strong foundation in chronic disease management, particularly diabetes, obesity, and metabolic syndrome.
Communication Skills: Exceptional interpersonal skills to communicate effectively with patients, families, and healthcare team members.
Team-Oriented: Ability to work collaboratively in a multidisciplinary environment and contribute to a supportive, patient-focused culture.
