SUMMARY
The Registered Nurses (RN) will supervise all Licensed Practical Nurses (LPN). The RN is responsible for implementing and monitoring individualized care plans in accordance with the instructions and clinical direction provided by the Physician for individuals residing in a supportive home environment. The RN is responsible for providing education and training to staff on proper care practices. The ideal candidate takes pride in providing high-quality, person-centered care and demonstrates excellent observation, communication, and clinical decision-making skills.
ESSENTIAL DUTIES
· Supervise all Licensed Practical Nurses (LPN).
· Medication Administration: Administer medications and monitor side effects as well maintaining the Medication Administration Records and completing follow-up on any identified issues.
· Medical Follow-Up: Review individuals’ medical records to ensure medical follow-up is completed which includes accompanying individuals to their medical appointments.
· Health Monitoring: Track vital signs and monitor the health conditions of residents.
· Care Coordination: Work with doctors and other healthcare providers to coordinate care plans.
· Education: Educate individuals and staff on managing their health.
· Compliance: Ensure the home complies with all health and safety regulations.
· Patient Care: Provide support for daily living activities, recovery from surgery or illness, and chronic disease management.
· Perform all essential nursing duties.
· Communicate effectively with residents, staff, guardians and healthcare providers.
· Ensure adherence to nursing standards and regulatory requirements.
· Promote a safe, respectful, and caring environment.
MINIMUM QUALIFICATIONS
· The RN will provide ongoing clinical oversight, including delegation of nursing tasks, review and approval of nursing documentation, monitoring of the LPN’s performance, and availability for consultation and guidance as needed. The RN will conduct regular supervisory reviews and evaluations to ensure quality of care, compliance with agency policies, and adherence to all regulatory requirements.
· Current and valid RN license (New Jersey).
· Experience in residential or long-term care is preferred, but will consider other nursing experience.
· Cooperate with licensing and department of human services staff in any inspection, inquiry, or investigation.
· Submit written acknowledgement and submit to a criminal background check and be determined to be not disqualified by the Department of Human Services prior to working with individuals.
· Submit written acknowledgement to have their name checked against the central registry of offenders against individuals with developmental disabilities.
· Submit written acknowledgement and submit to drug testing prior to employment.
· Submit written acknowledgement and submit to drug testing conducted randomly and for cause.
· Submit written acknowledgement and complete an application to have their name checked against the child abuse registry information (CARI).
· Have a Driver license valid in the state of New Jersey.
· Be at least 18 years of age.
· Strong clinical judgment and problem-solving abilities.
· Excellent communication and teamwork skills.
EDUCATION and EXPERIENCE
· High School Diploma or Equivalent
· Current and valid RN LICENSE (New Jersey)
