Job Responsibilities:

Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.

Collaborate with multidisciplinary team members to optimize patient care and outcomes.

Educate patients and their families on health maintenance and disease prevention.

Ensure accurate documentation and compliance with all regulatory guidelines.

Perform patient assessments and develop individualized care plans

Administer IV infusions and manage vascular access devices

Educate patients/caregivers on therapy, side effects, and self-care

Monitor for complications and respond appropriately

Collaborate with physicians, pharmacists, and home health teams

Travel to patients’ homes with reliable transportation

Requirements:

Active RN license in the state of practice.

Previous experience in a home health or acute care setting is advantageous.

Exceptional interpersonal and communication skills.
