Job Summary
We are seeking a clinically skilled and compassionate Registered Nurse (RN) to join our family‑owned home health and hospice organization on a per‑diem or part‑time basis, with the opportunity to transition into a full‑time role. The RN is responsible for providing skilled nursing services to patients in their place of residence, in accordance with physician orders, individualized plans of care, and regulatory standards.
This role includes the comprehensive assessment of patient needs, development and management of individualized plans of care, and coordination with the interdisciplinary care team to promote optimal patient outcomes. In the hospice setting, the RN provides symptom management, pain control, patient and family education, and emotional support, ensuring care is delivered with dignity, comfort, and respect at end of life.
The RN will monitor patient status, identify changes in condition, and communicate effectively with physicians, clinicians, patients, and family members. Accurate clinical documentation, compliance with state and federal regulations, and adherence to professional nursing standards are essential components of this role.
The ideal candidate demonstrates strong clinical judgment, critical thinking, and problem‑solving skills, along with the ability to work independently in a home‑based environment. Experience in home health or hospice nursing is preferred.

Responsibilities
Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
Administer medications and monitor patients for side effects or reactions
Document any changes in patient conditions and report on changes
Maintain accurate records of patient care and discuss observations with the supervisor
Qualifications
Currently licensed Registered Nurse (RN) in state of Utah
Previous experience as a Registered Nurse is preferred
Excellent interpersonal and client care skills
Valid driver’s license
First aid/CPR certification required
