    Your Role: Registered Nurse (RN)

    As a registered nurse under the nurse manager's or designee's direction, the nurse provides professional and skilled nursing care for individuals with developmental disabilities in the office, homes and community settings. You’ll encourage independence, promote engagement, and help clients achieve their personal goals with compassion and integrity.

    Key Responsibilities:

    Perform skilled nursing assessments during in-home and telehealth visits.

    Manage and monitor Type 1 diabetes care (CGM review, insulin oversight, hypo/hyperglycemia response).

    Perform and supervise G-tube feedings, assess tolerance, and manage feeding complications.

    Provide and evaluate special diets (carb-controlled, gluten-free) and nutritional status.

    Perform and manage wound care and skin integrity assessments.

    Perform and manage nursing respite.

    Provide caregiver teaching on diabetes care, tube feeding, wound care, and sick-day management.

    Assess fall risk, bowel patterns, urinary incontinence, and overall safety.

    Respond to remote monitoring alerts and determine need for in-person care.

    Communicate clinical changes to physicians and care teams.

    Document all nursing care and patient response.

    Chart behavior or condition observations on the computer

    Drive or assist the driver in safely transporting individuals with disabilities, ensuring comfort and support during transit

    Ensure safety and dignity in every interaction

    Carry out additional duties as directed by the supervisor

    Required Qualifications:

    Must be 18+ years old

    Active RN license

    Pediatric or home health experience preferred

    Experience with diabetes and enteral feeding preferred

    Comfortable with telehealth

    CPR and First Aid Certification

    Article 9 Certification (or willingness to obtain)

    Prevention & Support Certification (or willingness to obtain)

    Clean Level One Fingerprint Clearance Card (or ability to obtain)

    Able to pass a criminal background check

    Reliable transportation and willingness to travel locally

    Nice to Have:

    Experience working with individuals with developmental or intellectual disabilities
