Job Summary

Elite Care Home Health and Hospice is seeking a Registered Nurse to join our team. This is a per-visit position, ideal for an RN who wants control over their schedule while still doing meaningful, hands-on patient care. You will create and manage individualized plans of care for home health and hospice patients, working independently in patients' homes while staying connected to a supportive clinical team. You are detail oriented with a big heart, take pride in personalized care planning, and communicate effectively in your patient's best interest.

Responsibilities

Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans for home health and hospice patients

Administer medications and monitor patients for side effects or reactions

Document any changes in patient condition and report findings promptly

Maintain accurate, timely records of patient care and discuss observations with your supervisor

Accept PRN visits based on your own availability

Qualifications

Currently licensed Registered Nurse (RN) in the state of Texas

Previous home health or hospice experience preferred

Excellent interpersonal and patient care skills

Valid driver's license and reliable transportation for home visits

First aid/CPR certification required
