PM Shift – Full-Time Schedule: 6:00 p.m. – 6:30 a.m. Three shifts per week with an every-third-weekend rotation Position Summary: The Care Center RN delivers direct patient care within our Inpatient Care Center, providing compassionate, high-quality nursing support to patients and their families. This role emphasizes skilled symptom management, emotional support, and patient education while working collaboratively as part of an interdisciplinary hospice care team. Responsibilities: Deliver skilled nursing care to patients and families, including: • Inpatient Care Center • SIA/Continuous Care • Referrals and admissions • Chart reviews and other assigned duties • Follow established Plan of Care and physician orders; communicate changes as needed • Coordinate and participate in IDG planning and care review • Teach and supervise hospice aides, volunteers, and primary caregivers • Ensure smooth transitions between home care and inpatient care • Educate patients and families on hospice services and benefits • Manage physician orders and durable medical equipment (DME) requests • Complete all required documentation accurately and on time • Maintain clinical competence and provide weekend/evening support as needed Qualifications: Education & Experience • Graduate of an accredited School of Nursing • Current RN license in the State of Ohio • Minimum 2 years of experience in Medical/Surgical, Oncology, or Community Health Nursing • Hospice experience preferred Additional Skills and Requirements: • Valid driver’s license • Strong customer service and communication skills • Excellent attention to detail, organization, and documentation skills • Ability to work independently and follow instructions • Willingness to travel to other sites as needed (including in inclement weather) • Able to lift up to 25 lbs and assist with patient mobility (ambulation, transfers, positioning, etc.)
