About the Role:

Responsibilities:

Conduct comprehensive assessments of patients’ health and develop personalized care plans.

Administer medications and treatments as prescribed, ensuring compliance with safety standards.

Monitor patient progress and adjust care plans as necessary while maintaining accurate records.

Educate patients and their families about health management and care procedures.

Collaborate with interdisciplinary teams to ensure holistic patient care.

Provide emotional support and counseling to patients and families.

Participate in quality improvement initiatives to enhance patient care services.

Stay updated with the latest nursing practices and regulations.

Requirements:

Active Registered Nurse (RN) license in New Jersey.

Minimum of 2 years of nursing experience, preferably in home health care.

Strong clinical assessment and critical thinking skills.

Excellent communication and interpersonal skills.

Ability to work independently and as part of a team.

Compassionate demeanor with a focus on patient-centered care.

Basic Life Support (BLS) certification required.

Proficient in electronic health records (EHR) systems.
