Job Summary
A Home Healthcare Registered Nurse is responsible for delivering high-quality nursing care to patients in their homes. This role includes performing skilled nursing visits, managing patient care plans, monitoring patient health, educating patients and caregivers, and working closely with interdisciplinary healthcare teams to ensure the best outcomes.
The nurse must be able to assess a variety of medical conditions, provide compassionate care, and support the patient’s independence and quality of life. Home Healthcare Nurses must be self-directed, organized, and comfortable working independently while representing the agency with professionalism and empathy.
Responsibilities Include:
Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
Administer medications and monitor patients for side effects or reactions
Document any changes in patient conditions and report on changes
Maintain accurate records of patient care and discuss observations with the supervisor
Conducting in-home patient assessments and skilled nursing visits
Creating and updating individualized care plans
Collaborating with physicians, therapists, and caregivers
Educating patients and families on health management
Ensuring compliance with all clinical and safety standards
Requirements:
Currently licensed Registered Nurse (RN) in state of employment
Previous experience as a Registered Nurse is preferred
Excellent interpersonal and client care skills
Valid driver’s license
First aid/CPR certification required
