We are seeking an exceptional Registered Nurse (RN) who is passionate about chronic disease management, patient education, and building long-term relationships with complex, high-risk patients.

Key Responsibilities

• Support patients with advanced pulmonary and cardiopulmonary conditions

• Provide education, coaching, and longitudinal support

• Coordinate care across providers and settings

• Assist in managing high-risk populations

• Contribute to Chronic Care Management (CCM) initiatives

• Support transitional care and continuity across episodes

Clinical Background We Value

• Experience in ICU, Pulmonary clinic, Cardiology clinic, or Cardiopulmonary care

• Strong understanding of chronic disease management

• Experience in Chronic Care Management (CCM), Value-Based Care (VBC), Care Coordination, or Population Health preferred
