Overview

Hiring an experienced RN with strong home health background and hands-on OASIS experience. Must be independent, detail-oriented, and comfortable with in-home patient care.

Responsibilities

Perform Start of Care (SOC), Recertification, Resumption of Care (ROC), and Discharge OASIS

Conduct patient assessments and create care plans

Coordinate with physicians and care team

Monitor patient progress and update plans

Educate patients and families

Ensure Medicare compliance

Complete documentation accurately and on time

Requirements

Active RN license (Massachusetts)

1–2+ years home health experience

Strong OASIS experience (required)

Knowledge of Medicare guidelines

Valid driver’s license + reliable transportation

Preferred

Experience with Alora Plus

EMR documentation experience
