Job Summary

We are seeking a home health rehab aide to join our team! As a rehab aide for our physical therapist, you will be driving the physical therapist (PT) to each visit, providing routine support alongside the PT, and entering information from the visit into the electronic medical record. The ideal candidate has strong customer service skills and an interest in learning more about a working medical environment. Must be comfortable with significant amounts of driving throughout the metro. The home health rehab aide will be able to provide moderate physical exertion during a session to assist the physical therapist and/or patient with transfers. This is a field role involving driving, standing, stooping, walking, as well as documentation and telephone communication.

Responsibilities

Prepare for the patient visit, including pulling charts and notes

Take vitals and clean equipment after use

Moderate physical exertion during a session to provide physical assistance

Enter data into the medical chart during the physical therapist's visit

Complete administrative tasks as requested

Drive a company vehicle between visits

Qualifications

BLS Certification

Excellent customer service skills

Professional communication with patients, families, and staff

Ability to take detailed notes and accurate documentation

Strong computer and transcription skills

Travel is required to patient homes and facilities

Ability to adhere to strict confidentiality requirements

Must be comfortable driving throughout the Twin Cities metro
