As a Rehabilitation Support Worker, you will work directly with individuals who are navigating recovery, mental health challenges, and life transitions. You will play an important role in supporting clients with everyday activities, encouraging personal growth, and helping create a stable and supportive environment where individuals can succeed. Responsibilities: • Client Transportation: Provide reliable transportation for clients to and from appointments and community activities. • Facilitate Group Activities: Lead recovery-focused groups and activities that promote healing, skill-building, and personal growth. • Assessment & Evaluation: Identify barriers that may affect a client’s ability to live independently or integrate successfully into the community. • Communication Support: Assist clients in developing effective communication and interpersonal skills for daily interactions. • Symptom Management: Support clients in recognizing and implementing strategies for managing their symptoms and maintaining stability. • Social Support Development: Help clients strengthen and expand their social support networks. • Life Skills Training: Teach and reinforce daily living skills related to home, work, education, leisure, and community engagement. • Crisis Intervention: Provide immediate, compassionate support during crises to ensure client safety and emotional well-being. • Documentation: Maintain timely, accurate, and detailed documentation of all client interactions, progress, and interventions. • Team Collaboration: Participate in staff meetings, case consultations, and care coordination efforts. • Training & Development: Complete all required trainings (both in-person and online) to remain current with best practices and agency standards. • Productivity Standards: Consistently meet or exceed established productivity and performance expectations. Qualifications: Qualifications & Requirements • Education: High school diploma or GED required. • Age Requirement: Must be at least 18 years old (per licensing standards). • Driver’s License & Insurance: Must possess a valid driver’s license, reliable transportation, and current auto insurance. • Attendance: Maintain regular and dependable attendance to ensure continuity of client care. • HIPAA Compliance: Adhere to confidentiality requirements and all HIPAA regulations. Compensation: $16.50 hourly
