Manager of Case Management

Overview:
Reliatus Behavioral Health is seeking a dedicated and experienced Manager of Case Management to join our team in St. Paul, Minnesota. The ideal candidate will possess strong leadership skills, a passion for helping others, and the ability to effectively manage and support our case management team. This role requires a Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field.
Key Responsibilities:
\- Oversee the daily operations of the case management department, ensuring high-quality service delivery.
\- Supervise and support case managers, providing guidance and professional development opportunities.
\- Develop and implement case management policies and procedures to enhance service efficiency and effectiveness.
\- Ensure compliance with all relevant regulations and standards in case management practices.
\- Manage and resolve complex cases and issues, providing expertise and direction as needed.
\- Coordinate with other departments and external agencies to facilitate comprehensive care for clients.
\- Monitor and evaluate program performance, using data to inform improvements and innovations.
\- Prepare and present reports on case management activities and outcomes to senior management.
Qualifications
\- Bachelor’s Degree in Human Services, Social Work, Psychology, Sociology, or a related field.
(Social Workers must be licensed in the State of MN)
\- Proven experience in case management, with a minimum of 3-5 years in a supervisory or managerial role.
\- Strong understanding of case management principles, practices, and regulations.
\- Excellent leadership, organizational, and communication skills.
\- Ability to work effectively in a fast-paced and dynamic environment.
\- Proficient in using case management software and other relevant technologies.
