Job Description: We are looking for a highly skilled Nurse or Monitoring Specialist who will monitor patients' vitals from our local office, determine whether they are in or out of range, and communicate with patients and nursing staff as needed.

Responsibilities:

Monitor patient's vitals and health data using remote monitoring technology from our local office

Continuously monitor and evaluate patient progress and respond to alarms through remote monitoring tools

Collaborate with other healthcare providers to develop and implement individualized care plans

Identify and address potential health issues, providing timely interventions as needed

Maintain accurate and detailed patient records including any communication with the patient or other members of the care team

Qualifications:

Formal education and valid licensure as an RN or LPN

Strong verbal and communication skills in English

In-depth understanding of disease processes related to vital sign monitoring (HTN, CHF, CKD, COPD etc)

Assessment and critical thinking skills

Time management and prioritization skills

Technical proficiency and adaptability using digital health tools
