Job Title: Remote Patient Monitoring (RPM) Coordinator
Location: Medwin Clinic, Brownsville, Sugarland, and Houston TX Reports To: Aetos manager Type: Part Time | In-Clinic | Monday–Tuesday
Job Summary:
The RPM Coordinator plays a key role in identifying, enrolling, and supporting eligible patients for the clinic’s Remote Patient Monitoring program. This position is responsible for patient outreach, education, onboarding, and ongoing support to ensure compliance with Medicare and commercial RPM requirements. The ideal candidate is detail-oriented, compassionate, and comfortable with technology and patient communication.
Key Responsibilities:
Patient Identification & Recruitment
Collaborate with physicians and medical staff to screen patients during office visits or via outreach.
Educate patients about the benefits of RPM and obtain consent for participation.
Onboarding & Setup
Coordinate distribution of RPM devices (e.g., blood pressure cuffs, weight scales, glucometers).
Train patients on proper device usage in person or via phone/video.
Document patient consent and enrollment as per CMS guidelines.
Compliance & Documentation
Maintain accurate records of interactions, data uploads, and patient communications.
Qualifications:
High school diploma or equivalent (Associate or Bachelor’s in health-related field preferred)
1–2 years of experience in a clinical or patient-facing role
Excellent communication skills (both verbal and written)
Comfortable working with digital tools and patient health data
Bilingual (English + Spanish) Compulsory
