Overview
We are seeking an experienced Registered Nurse (RN) Care Manager to join our remote care management team. This role is responsible for coordinating patient care, providing education, assessing changes in condition, and escalating clinical concerns to the appropriate provider. The RN Care Manager serves as a primary point of contact for patients in a telehealth environment and plays a key role in supporting positive patient outcomes.
This position requires an active New York RN license and the ability to quickly adapt to a fast-paced remote care setting. Training will be approximately one week before assuming responsibility for a patient panel.
Key Responsibilities
• Manage a panel of patients and serve as a clinical resource for changes in condition and care needs
• Conduct telephonic patient assessments and provide ongoing education and support
• Monitor incoming patient alerts and respond appropriately based on established protocols
• Assess physiological and patient-reported data to identify potential concerns
• Escalate clinical issues and communicate patient updates to MDs, NPs, and PAs
• Support care coordination activities and collaborate with multidisciplinary care teams
• Document patient interactions accurately and in accordance with clinical standards
• Assist with care plan development and implementation
• Follow standing orders, clinical protocols, and escalation pathways
• Provide support across programs as needed to meet operational and patient care needs
Required Qualifications
• Active Registered Nurse (RN) license in New York
• Graduate of an accredited nursing program
• Minimum 2 years of recent acute care clinical experience
• Cardiac care or heart failure management experience
• At least 6 months of telephonic triage, care management, or patient education experience
• Strong critical thinking and clinical assessment skills
• Excellent verbal and written communication skills
• Ability to work independently in a remote environment
• Comfortable using multiple technology platforms and telehealth tools
Preferred Qualifications
• Multi-state RN license Compacted
• Additional RN licensure in Texas, Hawaii, or Connecticut
• Bilingual English/Spanish
• Experience with remote patient monitoring, telehealth, or care management programs
