    Overview

    We are seeking experienced and motivated nurses to join our growing healthcare team providing Remote Chronic Care Management (CCM) services to patients across the United States. This is a fully remote role supporting U.S.-based patients and requires an active U.S. nursing license.

    Mandatory Requirements

    Active, unrestricted U.S. nursing license (RN, BSN, or LPN)

    Minimum 1–2 years of clinical experience (CCM experience preferred)

    Strong English communication skills (verbal and written)

    Proficiency with telehealth platforms and EMR/EHR systems

    Reliable high-speed internet and a quiet, professional workspace

    Key Responsibilities

    Conduct remote patient monitoring and chronic care management outreach

    Communicate with patients via phone and digital platforms

    Accurately document all patient interactions in EMR systems

    Support care plan adherence and provide patient education

    Collaborate with U.S.-based healthcare providers and care teams

    Preferred Qualifications

    Experience within the U.S. healthcare system

    Prior telehealth or remote patient care experience

    Familiarity with Medicare CCM programs
