POSITION OVERVIEW
Develop and utilize in-depth knowledge of the ImpactCare program to effectively support member engagement and their quality of life.
HOW YOU'LL CONTRIBUTE
- Meet call volume and enrollment requirements; proactively assess and revise approach as needed or directed.
- Utilize and share approved educational materials in a manner that empowers and educates current and potential members.
- Engage with existing and potential members in a way that supports enrollment and fosters awareness of our program.
- Work collaboratively with each member, their support system, and health care professionals to appropriately address questions and provide additional information in a manner that supports quality health outcomes and cost-effective utilization.
- Appropriately document all outreach efforts in a way that promotes departmental transparency and complies with HIPAA.
- Appropriately document health data which includes identifying potential cost containment opportunities; refer high-risk or changes in status situations to RNs.
- In conjunction with designated RN, create individualized, goal-directed, and member-centered care plans that support each member's ability to improve their health and quality of life.
- Autonomously create individualized, goal-directed, and member-centered health plans for lower risk members.
EXPERIENCE, EDUCATION, & CERTIFICATIONS YOU'LL NEED
- Must have an active LPN unrestricted license in a compact state and in good standing.
- At least one year of outbound call experience providing support to members (patients).
- At least three years of experience providing LPN support to individuals with chronic conditions.
- Experience working in a remote environment.
- Familiarity with Microsoft products such as Outlook, Word, Excel, and Teams.

- Superb written and oral communication skills, which includes the ability to adapt the approach and verbiage used depending on the audience.
- Self-motivated with the ability to overcome obstacles.

- Knowledge of different learning styles and the ability to leverage that knowledge.
