    PURPOSE: The Intake Specialist’s primary objective is to thoroughly and accurately complete intake information on guests presenting to the Rescue Mission’s Men’s Shelter for overnight stays in a timely manner. The intake process includes obtaining demographic information as well as information re: their reasons for seeking shelter and personal history. The Intake Specialist documents an initial assessment of the guest’s needs and provides the information to the shelter staff and case manager. He/she is responsible for scheduling the guest's first meeting with their case manager. The Intake Specialist is often the first person the guest engages in conversation with and must be welcoming to the guests and knowledgeable of policies of procedures of the Shelter as well as Mission culture to provide answers to their questions. This position reports to and is supervised by the Men’s Shelter Manager.

    10. Understanding of volunteer engagement and ability to utilize and appreciate volunteers in daily tasks.

    JOB DUTIES:

    | Responsibilities shall include but are not limited to the following: | % of Time | 1. | Complete intakes for guests presenting to the Rescue Mission’s Men’s Shelter: · All updates are entered into the HMIS database system · An assessment is documented at the end of the intake · Consent Forms are printed, read to the guest, explained and signed. This includes the HMIS Pathways Form. If a guest refuses to sign the HMIS form, the Chief Administrative Officer is notified immediately via email. · The Financial Policy is read to the guest, explained, and signed. · The Maintenance policy is read to the guest, explained and signed. · Shelter “Rules at a Glance” are read to the guest and explained · Copies of all consent forms, policies and procedures are provided to the guest. A copy of the intake document may be provided to the guest if requested · An appointment is scheduled with the Case Manager. Provide guest with Case Manager business card with appointment noted. · A chart is opened in the V-drive. The assessment notes are charted in the case notes section of this file. · The assessment is emailed to shelter staff. · Partial intakes are not acceptable. If new intakes have not present, compare the list of incomplete intakes with the evening sign in sheet and meet to complete intakes. If no incomplete intakes are present, call the WCC and let them know you are available to assist with their intakes if needed. | 80% | 2. | Provide support to Men’s Shelter Staff as available. This would include Dining Room supervision or Chapel supervision; however, intakes take priority and may be scheduled during these times. | 10% | 3. | Attend the following standing meetings: · Weekly Case Management Team Meeting · Weekly Men’s Shelter Staff Meeting · Monthly Case Management Training Meeting | 10% | 4. | Volunteer Engagement is a strategy that builds the capacity of the Rescue Mission through employee and volunteer partnership. It is built on the development and implementation of high-impact, meaningful volunteer opportunities that create positive outcomes for the organization and the volunteer. The Intake Specialist will often work with Case Management interns from area colleges and universities (considered volunteers) as well as volunteers in the Men’s Shelter, Dining Room and Chapel. | Ongoing | 5. | Other duties as assigned. | Ongoing
