Resident Care Coordination:
Develop and implement resident care program that meets the needs of all residents within parameters of company policies and governmental regulations.
Assess residents’ and potential residents’ needs and preferences through observations, interviews, and health assessments.
Coordinate medication, hospitalization, physician visits and/or appointments, including transportation. Develop, implement, and monitor individualized care plans in collaboration with the healthcare team and family members.
Assist Executive Director with potential resident screening and required intake documents in accordance with company policies and procedures and governmental regulations. Coordinate new resident orientation process and assist with resident move-ins.
Manage and assist with daily living activities, promoting resident independence and dignity.
Ensure that all care plans are up-to-date and reflect any changes in the resident’s condition or preferences.
Nursing Duties:
Provide direct nursing care, including administering medications, wound care, and other treatments in accordance with physician orders.
Serve as a liaison between residents, families, and healthcare providers to promote clear communication and address any concerns.
Conduct regular meetings with staff to discuss resident needs, care plans, and any changes in health status or behavior.
Collaborate with physicians, social workers, dietary staff, and other healthcare professionals to ensure comprehensive care delivery.
Participate in case management meetings and quality improvement initiatives to enhance resident care and services.
Monitor resident health status and document vital signs, symptoms, and responses to interventions.
Supervise and perform, when required, medication administration.
