Job Summary
We are seeking a Resource Navigator to join our team. In this role, you will work collaboratively with caregivers of individuals with dementia. The Resource Navigator plays a critical role in supporting individuals by identifying, coordinating, and facilitating access to essential services and resources that promote stability, self-sufficiency, and long-term success. This position serves as a frontline liaison between participants and community-based, government, and institutional service providers. The Resource Navigator ensures participants receive comprehensive, client-centered support by addressing barriers related to employment, education, housing, health, and social services. This role is essential in advancing equitable outcomes and improving participant engagement, retention, and success across programs. You must be available to work during normal business hours 9:00a-6:00p.
Responsibilities
Conduct comprehensive intake assessments to identify participant needs, goals, and barriers
Develop individualized service plans aligned with participant objectives
Build rapport and maintain consistent communication with participants
Identify and connect participants to appropriate community resources, including:
Workforce training and employment services
Housing and shelter support
Behavioral and physical health services
Financial assistance and public benefits
Maintain an up-to-date database of local and regional service providers
Facilitate warm handoffs and follow-ups to ensure successful service connections
Monitor participant progress and adjust service plans as needed
Document all interactions, referrals, and outcomes in case management systems
Ensure compliance with program requirements and reporting standards
Advocate on behalf of participants with service providers and partner agencies
Identify systemic barriers and escalate issues to program leadership
Support participants in navigating complex systems such as healthcare, workforce programs, and public assistance
Develop and maintain relationships with community organizations, employers, and service providers
Participate in cross-agency meetings and case conferencing
Support outreach and engagement efforts to expand service access
Track participant outcomes, service utilization, and referral success rates
Prepare reports for internal and external stakeholders
Ensure accuracy and confidentiality of participant records
Qualifications
Previous experience as a Care Coordinator or in a similar position is preferred
Strong problem-solving and organizational skills
Ability to manage multiple projects or tasks and prioritize appropriately
Ability to work in fast-paced situations and make sound decisions quickly
Excellent interpersonal skills and high level of compassion
Strong verbal and written communication skills
