As the Nurse Liaison, you will be a Nurse first and a Community Outreach Coordinator second with all of your outreach efforts being associated with each new client and with shared community partners or associated entities such as Hospitals, Rehab Centers, Senior Living Communities, Aging Life Care Managers, Home Health Therapists, and Physicians.
This position is offers a flexible work schedule as well as a mixed work location. Much of the nursing responsibilities can be managed virtually with the occasional in-person assessment being required either in clients homes or in various health care settings such as Hospitals, Rehab Centers, or Senior Living Communities. The community outreach efforts will also be managed remotely though phone calls, emails, and zoom meetings with the occasional in-person meetings when appropriate.
