HHA Certificate or CNA license

Level 2 Background w/ finger prints

Palm Beach County Caregiver Badge (if HHA)

Valid Driver's License

Auto Insurance

CPR Certified

TB or Chest X-ray

Alzheimer's certificate (2 hours training required)

HIV certificate

Assisting with Self-Administration of Medications certificate
